Before you send
Name the holder. Say what it is for.
- What to include
- The certificate holder’s legal name and address, and what the document must speak to — a venue, a landlord, an event, or a contract.
- What a certificate is
- A certificate or evidence of insurance shows coverage in force. It does not change the policy, add an insured, or bind new coverage.
- Where it goes
- The request goes to the certificate desk. Include a reply address so the document can be sent back.
