- Officers, governing bodies, advisors, appointed professionals, and brokers acting for those parties.
- Claims notice
- Select “Claims notice” as the topic and include the policy reference, the date you first knew of the matter, and a short factual account. Do not attach medical records.
- Certificates
- Use Request COI/EOI and name the holder and the contract or location. Certificates evidence coverage; they do not change it.
